Jim is the Founder and CEO of Revitalize Health and Wellness LLC. He is also a Board Certified NP practicing in Wisconsin, Georgia and Minnesota. In addition to his clinical practice, he is a Visiting Professor for multiple universities across the United States.

My Story

I’m Jim, founder of this clinic and a proud lifelong resident of the Stateline area. Growing up in rural Wisconsin, I saw firsthand the challenges our communities face when it comes to accessing quality healthcare. Too often, advanced medical services are only available in large cities—or worse, online platforms where you may never speak directly with your provider. I created this clinic to change that.

My journey in healthcare began in a Level 1 trauma center in Rockford, IL, and eventually led me to rural primary care. With over 15 years of medical experience, I grew increasingly frustrated with a system that limited care based on insurance rather than patient needs. That’s why I’ve built a practice focused on finding the root cause of your concerns, not just treating the symptoms. Here, we work together to build a personalized treatment plan designed to improve your overall quality of life.

Outside the clinic, I’m a proud father of five amazing kids. We love to spend time outdoors—hiking, biking, camping, and exploring the great state of Wisconsin in our camper during the summer months. Family and nature keep me grounded, and I bring that same heart and dedication into the care I provide.

Many people ask why we don't take insurance and the answer is simple: we can provide better care without interference and restrictions from insurance companies. Oftentimes, insurance companies will decline care as they deem it not a medical necessity and due to costs. This is even more true for those who are denied for weight-loss and testosterone replacement therapies!
